Medicare Fee-For-Service Beneficiaries

Our goal is to support your doctor in providing you with coordinated care. Medicare Accountable Care Organizations (ACOs) help your doctor and healthcare providers to work together more closely by making sure they have the most up-to-date information. Doctors will communicate with each other, helping you to avoid unnecessary test duplication or answering the same question on multiple visits. If your doctor chooses to participate in ApolloMed’s Medicare ACO, you will be notified both in person and by letter.

We will provide you with coordination of care and access to additional resources. We will help coordinate your physician visits, hospital stays, and other ancillary services such as physical therapy or lab tests. We also offer other resources such as care reminders, patient education, and support for individuals that must transition from their homes to the hospital.
